Support Coordination

Service Description

Support coordination is expected to provide participant's with training and support to implement their plan and access appropriate supports. Support coordinators assist participants to reduce complexity in their support environment, and overcome barriers to connecting with broader systems of supports as well as funded supports.

Cancellation Policy

To cancel or reschedule appointment please provide 6 hours notification. Late Cancellation fee will apply as per NDIS Price Guide.
