Mutual Responsibility

 

Our responsibilities as your provider

 

Your NQ Enable Support Coordinator agrees to:

  • communicate openly and honestly in a timely manner

  • treat you with courtesy and respect

  • consult you on decisions about how supports are provided

  • give you information about managing any complaints disagreements

  • listen to your feedback and resolve problems quickly

  • give you reasonable notice if NQ Enable must change a scheduled appointment to provide supports

  • take precautions to protect your privacy and confidential information

  • provide supports in a manner consistent with all relevant laws, including the National Disability Insurance Scheme Act 2013 and rules, and the Australian Consumer Law

  • keep accurate records on the supports provided

  • Issue regular invoices and statements of the supports delivered as agreed

 

Your responsibilities as a participant

 

You agree to:

  • tell your Support Coordinator about how you wish the supports to be delivered to meet your needs

  • treat your Support Coordinator with courtesy and respect

  • talk to your Support Coordinator if you have any concerns about the supports being provided

  • give your Support Coordinator a minimum of 24 hours' notice if you cannot make a scheduled appointment

  • give your Support Coordinator the required notice if you to end the Service Agreement

  • let your Support Coordinator know immediately if your NDIS plan is suspended or replaced by a new NDIS plan or you stop being a Participant in the NDIS

  • provide your Support Coordinator with any medical, health or other information that might be relevant to the providing supports and/or services to you
