Mutual Responsibility
Our responsibilities as your provider
Your NQ Enable Support Coordinator agrees to:

communicate openly and honestly in a timely manner

treat you with courtesy and respect

consult you on decisions about how supports are provided

give you information about managing any complaints disagreements

listen to your feedback and resolve problems quickly
-
give you reasonable notice if NQ Enable must change a scheduled appointment to provide supports

take precautions to protect your privacy and confidential information
-
provide supports in a manner consistent with all relevant laws, including the National Disability Insurance Scheme Act 2013 and rules, and the Australian Consumer Law

keep accurate records on the supports provided

Issue regular invoices and statements of the supports delivered as agreed
Your responsibilities as a participant
You agree to:

tell your Support Coordinator about how you wish the supports to be delivered to meet your needs

treat your Support Coordinator with courtesy and respect

talk to your Support Coordinator if you have any concerns about the supports being provided

give your Support Coordinator a minimum of 24 hours' notice if you cannot make a scheduled appointment

give your Support Coordinator the required notice if you to end the Service Agreement
-
let your Support Coordinator know immediately if your NDIS plan is suspended or replaced by a new NDIS plan or you stop being a Participant in the NDIS
-
provide your Support Coordinator with any medical, health or other information that might be relevant to the providing supports and/or services to you
