Support Coordination assists Participants to:

Understand the NDIS Plan

  • Explain the NDIS plan and funding categories

  • Clarify participant goals and priorities

  • Break down plan budgets (Core, Capacity Building, Capital)

Plan & Goal Setting

  • - Develop an action plan to implement the NDIS plan

  • - Set clear, measurable goals

  • - Prioritise urgent or high-need supports

Connect with Services

  • - Link participant with NDIS providers, community & mainstream supports

  • - Support with choosing providers and booking services

  • - Assist with Service Agreements & understanding T&Cs

Support Implementation

  • - Coordinate service delivery across providers

  • - Monitor budget use & provider invoices

  • - Troubleshoot service issues or changes

Crisis Management (if required)

  • - Identify risks and plan for emergencies

  • - Coordinate urgent support responses

  • - Work with mainstream crisis or health services

Monitor & Review Supports

  • - Track progress toward goals

  • - Regular check-ins on service effectiveness

  • - Assist with Plan Reviews & NDIA reporting

Capacity Building

  • - Support decision-making and self-advocacy

  • - Build skills in budgeting, problem-solving, service navigation

  • - Encourage independence and confidence

Documentation & Reporting

  • - Maintain accurate case notes

  • - Complete all NDIA reports and evidence for plan reassessments

  • - Ensure privacy and data compliance

Choice, Control & Safeguarding

  • - Promote participant's choice and control

  • - Respect diversity, culture, and values

  • - Identify and respond to safeguarding concerns​​
